Menopause: Black Cohosh, Sage, Hot Flashes & Evidence
Menopause products often get sold as if every herb works the same way. They do not. Product identity, extract type, dose, and the exact symptom being studied all matter.

NCCIH reports that some black cohosh products may improve overall menopause symptoms and hot flashes, but study results have been inconsistent. A few small studies suggest common sage may reduce hot-flash frequency, but the evidence base is much smaller.
Black cohosh
A 2023 review of 22 studies found potential benefit for overall menopause symptoms and hot flashes, while other reviews have found inconsistent results. Rare cases of serious liver injury have been reported in people using products labeled as black cohosh.
Sage
A small amount of research suggests common sage may reduce hot-flash frequency, but there is not enough evidence for firm conclusions. High doses or long-term use may be unsafe because some sage species contain thujone.
